The most common causes are a missing or incorrect <link rel="icon"> tag in your page's <head>, an aggressively cached old favicon in the visitor's browser, or the favicon file itself returning a 404.
A 32x32px or 16x16px .ico or .png file works for most browsers and tabs. For better support across devices (including Apple touch icons), consider also adding a 180x180px PNG.
Not strictly - modern browsers respect a <link rel="icon"> tag pointing to any image path or format (including .png and .svg). The /favicon.ico location is only used as a fallback if no link tag is found.
